CAMERA VIEW
INSTRUCTIONS:
Ask the patient:
1. What is the ‘current’ condition of the your teeth?
2. Do you wear dentures?
3. Do your teeth / dentures bother you ‘now’.
4a. Do you notice any movements in your mouth, face, hands, or feet?
4b. If yes, please describe them and indicate to what extent they ‘currently’ bother you or interfere with your activities.
Seated Full Body
CAMERA VIEW
INSTRUCTIONS:
ALIGN TO OVERLAY.
Have the patient sit in the chair with hands on knees, legs slightly apart, and feet flat on floor. Observe for 20 seconds.
Hanging Hands
CAMERA VIEW
INSTRUCTIONS:
Ask the patient to sit with hands hanging unsupported – hanging over patient's knees. Video for 20 seconds.
Hand Tapping
CAMERA VIEW
INSTRUCTIONS:
Leave hands hanging over knees. Ask the patient to tap his or her thumb with each finger as rapidly as possible for 10 to 15 seconds, first with right hand, then with left hand.
Arm Flex
CAMERA VIEW
INSTRUCTIONS:
While seated. Flex and extend the patient’s left and right arms, one at a time.
Mouth
CAMERA VIEW
INSTRUCTIONS:
CHANGE - ALIGN TO OVERLAY.
Ask the patient to open their mouth. Do this twice.
Ask the patient to protrude their tongue. Do this twice.
Stand up
CAMERA VIEW
INSTRUCTIONS:
CHANGE - ALIGN TO OVERLAY.
Ask the patient to stand up.
Arm Extensions
CAMERA VIEW
INSTRUCTIONS:
CHANGE - ALIGN TO OVERLAY.
Ask the patient to stand up.
Walking
CAMERA VIEW
INSTRUCTIONS:
CHANGE - ALIGN TO OVERLAY
Walk away from patient - align feet in the start box. Camera parallel to patient.
Have patient take steps, shoulders clearly visible and reflectors on front of feet. Ask patient to walk and instruct to stop before feet leave frame.
